针刺蝶腭神经节治疗主观耳鸣临床研究
Clinical Study on Needling Sphenopalatine Ganglion in Treatment of Subjective Tinnitus

Abstract
Objective:To compare the therapeutic efficacy of needling sphenopalatine ganglion versus conventional acupuncture in treatment of subjective tinnitus,and to explore the correlation between tinnitus and TCM body constitution.Methods:60 patients with subjective tinnitus were randomly assigned to the treatment group(n=30)and the control group(n=30).The treatment group was treated with needling sphenopalatine ganglion,whereas the control group was treated with conventional acupuncture.Both groups were treated once weekly for 6 weeks.The Tinnitus Handicap Inventory(THI)score was compared between the two groups before and after the treatment.In addition,an exploratory analysis was conducted to assess the effect of needling sphenopalatine ganglion on the average hearing loss in patients with subjective tinnitus.A correlation analysis was also performed between TCM body constitution types and THI scores.Results:The THI score in the treatment group was significantly lower than that in the control group after the treatment(P＜0.01).Compared with baseline,the treatment group showed a significant reduction in the THI score after 6 weeks of acupuncture(P＜0.01),whereas no significant change was observed in the control group(P＞0.05).After 6 weeks of treatment,the average hearing loss in the treatment group was significantly reduced compared with the control group(P＜0.05).Among the 60 tinnitus patients,the most common TCM body constitution types were phlegm-dampness(15 cases,25％),gentleness(9 cases,15％),Yang-deficiency(8 cases,13.3％),and Qi-stagnation(7 cases,11.7％).Correlation analysis revealed that the THI score was significantly associated with gentleness constitution(P=0.014),Yin-deficiency constitution(P=0.003),blood-stasis constitution(P=0.003),and Qi-stagnation constitution(P=0.002),but showed no significant correlation with other constitutions(P＞0.05).Conclusion:Needling sphenopalatine ganglion demonstrates superior efficacy over conventional acupuncture in alleviating the impact of tinnitus on patients' quality of life.关键词
