Abstract
Objective:To explore the application value of targeted high-throughput sequencing(tNGS)technology for bronchoalveolar lavage fluid(BALF)in the etiological diagnosis of hospital-acquired pneumonia(HAP).Method:A total of 184 patients with HAP admitted to Shantou Hospital of Traditional Chinese Medicine from October 2023 to October 2024 were selected as the research subjects.Based on bronchoalveolar lavage fluid(BALF),the differences in pathogen distribution detected by tNGS and traditional microbial culture were compared,and the differences in diagnostic efficacy of the two methods for mixed infections were further analyzed.Result:The total positive rate of tNGS detection was higher than that of traditional microbial culture(79.35%),but the consistency of the results between the two methods was low,and the difference was statistically significant(P<0.05).The detection rates of tNGS in bacteria and fungi(79.91%,66.85%)were higher than those in traditional microbial cultures(57.07%,32.07%),and the difference was statistically significant(P<0.05).Among 184 patients with HAP,93 were single infections,79 were multiple infections,and 12 had no clear pathogen.Among the mixed infections,the most common infection pattern was bacteria-bacterial infection(45.6%,36/79),followed by bacteria-fungal infection(27.85%,22/79)and bacteria-viral infection(10.1%,8/79).Conclusion:tNGS is superior to conventional microbial culture in the etiological diagnosis of BALF in patients with HAP.It can be used as a supplementary test to traditional microbial culture to provide diagnostic basis for patients with HAP.关键词
