Abstract
Objective:To analyze the application effect of ultrasound-guided percutaneous transhepatic cholecystectomy and catheterization(PTGBD)in patients with high-risk acute cholecystitis.Method:A total of 102 patients with high-risk acute cholecystitis admitted to the Second Affiliated Hospital of Bengbu Medical University from October 2022 to March 2025 were selected as the research subjects.According to the patients'wishes and different surgical methods,they were divided into the conventional group(n=46)and the observation group(n=56).The conventional group underwent laparoscopic cholecystectomy(LC),while the observation group underwent ultrasound-guided percutaneous transhepatic cholecystectomy and catheterization(PTGBD)+LC.The surgical-related indicators,liver function,inflammatory response indicators and the occurrence of complications were compared between the two groups.Result:Compared with the conventional group,the LC operation time,hospital stay and conversion to laparotomy rate in the observation group were all lower(P<0.05).Before the operation,the levels of total bilirubin(TBiL),aspartate aminotransferase(AST),and alanine aminotransferase(ALT)in the two groups were close(P>0.05).Compared with the preoperative level,the levels of TBiL,AST and ALT in both groups decreased 3 days after the operation.Moreover,compared with the conventional group,the liver function index levels in the observation group were lower 3 days after the operation(P<0.05).Before the operation,the levels of procalcitonin(PCT),C-reactive protein(CRP),and interleukin-6(IL-6)in the two groups were close(P>0.05).Compared with before the operation,the levels of PCT,CRP,and IL-6 in both groups decreased 3 days after the operation.Moreover,compared with the conventional group,the levels of inflammatory response indicators in the observation group were lower 3 days after the operation(P<0.05).The total incidence of complications in the conventional group was higher than that in the observation group(P>0.05).Conclusion:Ultrasound-guided PTGBD can effectively alleviate the inflammatory response in patients with high-risk acute cholecystitis,promote their recovery,and has a low risk of complications,making it safe.关键词
