Abstract
Objective:To explore the pathogenic characteristics and drug resistance analysis of pulmonary infections in critically ill pneumonia patients undergoing mechanical ventilation in the intensive care unit(ICU).Method:A total of 253 severe pneumonia patients who underwent mechanical ventilation treatment in the ICU of the Xiamen Xinglin Hospital from July 2023 to June 2024 were selected as the research subjects.Sputum samples were collected and subjected to microbial culture and drug sensitivity testing.The distribution of pathogenic bacteria in the patients with pulmonary infection,mainly including Gram-positive bacteria,Gram-negative bacteria,and Fungi,was statistically analyzed,and the drug resistance of the above-mentioned pathogens was analyzed.Result:A total of 320 strains of pathogenic bacteria were isolated from 253 ICU mechanically ventilated severe pneumonia patients,including 65.94%(211/320)of Gram-negative bacteria,27.81%(89/320)of Gram-positive bacteria,and 6.25%(20/320)of Fungi.The drug sensitivity results showed that among Gram negative bacteria,Acinetobacter baumannii and Pseudomonas aeruginosa had the highest resistance rates to gentamicin and levofloxacin,while the resistance rates to imipenem were the lowest.Klebsiella pneumoniae had the highest resistance rate to aztronam and the lowest resistance rate to meropenem.Gram positive bacteria Staphylococcus aureus and Streptococcus pneumoniae had the highest resistance rates to penicilli.No resistant strains to linezolid were detected in both Gram positive bacteria.Among the fungi,the number of Candida strains resistant to fluconazole and amphotericin B was 5 and 2 respectively,and the resistance rates were 33.33%and 13.33%,respectively.Conclusion:Gram negative bacteria are the main pathogens causing pulmonary infections in critically ill pneumonia patients undergoing mechanical ventilation in the ICU,and there are significant differences in drug resistance among different pathogens.Therefore,it is necessary to strengthen pathogen testing to ensure targeted and rational use of antibiotics.关键词
