Tongdu Tiaoshen Electroacupuncture Improves Insomnia Patients' Diffusion Tensor Imaging Along Perivascular Space by Regulating Glymphatic Function

Abstract
Objective To investigate the brain mechanisms of Tongdu Tiaoshen electroacupuncture in treating insomnia disorder(ID)by assessing the Diffusion Tensor Imaging along the Perivascular Space(DTI-ALPS)index.Methods Twenty ID patients received Tongdu Tiaoshen electroacupuncture at Baihui(GV20),Yintang(GV29),bilateral Shenmen(HT7),and Sanyinjiao(SP6),with electrical stimulation(30 min/session,every other day,12 sessions total).The Pittsburgh Sleep Quality Index(PSQI),Hyperarousal Scale(HAS),and portable polysomnography(PPSG)parameters were evaluated,and DTI scans were performed to calculate the ALPS index before and after treatment.A demographically matched healthy control(HC)group was recruited to establish baseline data.Comparisons were made between pre-and post-treatment outcomes in the ID group,as well as for ALPS differences between the ID and HC groups.Stratified and correlation analyses were also conducted.Results After treatment,ID patients showed significant reductions in PSQI and HAS scores(P<0.01),increased total sleep time(TST),sleep efficiency(SE),and NREM 3%(P<0.05),and decreased sleep latency(SL)and wake after sleep onset(WASO)(P<0.05).Baseline ALPS values were lower in the ID group than in the HC group(P<0.05).Although the post-treatment change in ALPS was not statistically significant,a slight increase was observed(A=0.04).Patients with high baseline ALPS showed significant improvement after treatment(P<0.05).ALPS values were positively correlated with SE(r=0.394)and negatively correlated with WASO(r=-0.409).Conclusions Tongdu Tiaoshen electroacupuncture may improve ALPS and sleep parameters in ID patients by modulating brain glymphatic system,providing multimodal neuroimaging evidence for its central mechanism of action.关键词
