Abstract

Macrophages can differentiate into distinct phenotypes under various stimuli,thereby exerting diverse functions,a process known as macrophage polarization.This process plays a key role in multiple pathophysiological conditions.Specifically,M1 macrophages predominantly mediate pro-inflammatory responses and tissue injury,whereas M2 macrophages are associated with anti-inflammatory effects and tissue repair.Recent studies have demonstrated that macrophage polarization is critically involved in the onset and progression of kidney diseases.In the early stages of the disease,infiltration is mainly characterized by M1 macrophages,and as the disease advances,macrophages gradually shift toward the M2 phenotype.Among these,M1 macrophages are closely linked to kidney injury,while M2 macrophages contribute substantially to renal fibrosis.However,due to the complexity of macrophage polarization and the heterogeneity of kidney disease pathogenesis,its precise mechanisms in renal disorders remain incompletely understood.Macrophage polarization is tightly regulated by multiple signaling pathways,including stimulator of interferon genes(STING),signal transducer and activator of transcription(STAT),mitogen-activated protein kinase(MAPK),and Notch,as well as key mediators such as interferon-γ and NOD-like receptor thermal protein domain associated protein 3(NLRP3).Interventions targeting this regulatory network,such as suppressing M1 polarization,may confer anti-inflammatory and renoprotective benefits;in contrast,modulation of M2 polarization requires individualized strategies tailored to disease type and stage.In summary,systematically delineating the factors influencing macrophage polarization across various kidney diseases and elucidating the functional roles of different phenotypes may offer new perspectives for developing therapeutic strategies for renal disorders.
