Best evidence summary for the management of autologous arteriovenous fistula during the initiation phase with ultrasound guidance
摘要
Abstract
Objective To retrieve,evaluate and summarize the best evidence for ultrasound-assisted puncture in the initia-tion phase of autologous arteriovenous fistula(AVF).Methods By systematically searching relevant databases at home and abroad,the relevant clinical evidence on the management of ultrasound-guided AVF in maintenance hemodialysis patients during the initiation phase was comprehensively collected.The types of included literature included guidelines,systematic reviews,expert consensus,randomized controlled trials and other high-quality research evidence.The search period was from January 2014 to December 2024.The evidence-based research team was responsible for literature screening,quality evaluation,evidence extraction and summary of evidence.Results A total of 16 articles were included,including 2 clinical decisions,6 guidelines,3 expert consensus and 5 randomized controlled trials.A total of 16 pieces of evidence were summarized,including the evaluation criteria of fistula maturity,preparation before puncture,precise operation rules of puncture and management strategies after puncture.Conclusion The best evidence summary of ultrasound-guided AVF puncture in the initiation phase based on the characteristics of Chinese patient population,such as body size differences and clinical resource distribution,promotes the localized optimization of the key evidence of blood flow threshold and initial blood pump flow setting in the mature ultrasound standard of AVF,which can be used as a guide and specification for clinical nurses to implement ultrasound-guided AVF puncture in the initiation phase.To reduce the incidence of puncture related complications,prevent long-term complications,and ensure the safety of patients.关键词
