腹腔镜与开放式经腹腹膜前疝修补术治疗成人腹股沟疝的成本-效果分析
Cost-effectiveness analysis of laparoscopic and open transabdominal preperi-toneal in the treatment of adult inguinal hernia

Abstract
Objective To explore the cost-effectiveness of laparoscopic and open transabdominal preperitoneal(TAPP)for adult inguinal hernia.Methods A total of 102 adult patients with inguinal hernia admitted to Fengcheng Hospital of Tradi-tional Chinese Medicine from October 2023 to October 2024 were selected as the research subjects and they were divided into the experimental group(51 cases)and the control group(51 cases)according to the random number table method.The control group was treated with transperitoneal hernia repair(Kugel)surgery,and the experimental group was treated with laparoscopic TAPP surgery.The perioperative indicators,visual analogue scale(VAS)scores,complications,recurrence control and cost-related indicators of the two groups were compared.Results The duration of getting out of bed and the length of hospital stay in the experimental group were shorter than those in the control group,the intraoperative blood loss was less than that in the control group,and the operation duration was longer than that in the control group,the differences were statistically significant(P＜0.05).There were statistically significant differences in the VAS score time,between groups,and interaction between the two groups(P＜0.05).The VAS scores of the experimental group at 24 hours,1 month,and 6 months after surgery were lower than those of the control group,and the differences were statistically significant(P＜0.05).The incidence of complications in the experimental group was lower than that in the control group,and the recurrence rate was also lower than that in the control group,with statistically significant differences(P＜0.05).The combined rate of direct cost in the experimental group was higher than that in the control group,while the indirect cost,cost-effect,total cost were lower than those in the control group,the differences were statistically significant(P＜0.05).The quality-adjusted life year of the experimental group was higher than that of the control group,the incremental cost-effectiveness ratio was lower than that of the control group,and the differences were statistically significant(P＜0.05).Conclusion In the treatment of adult inguinal hernia,both laparoscopy and Kugel surgery can achieve certain therapeutic effects.However,TAPP surgery has fewer complications,shorter hospital stays and ambulation periods,less postoperative pain,and better cost-effectiveness.关键词
