老年射血分数降低心力衰竭患者心电指标Tp-Te间期与左室射血分数的相关性及对恶性心律失常的联合预测价值分析
Correlation between Tp-Te interval of electrocardiographic index and left ventricular ejection fraction in elderly patients with heart failure with re-duced ejection fraction and analysis of the combined predictive value of malignant arrhythmia

Abstract
Objective To investigate the correlation between the electrocardiographic index Tpeak-Tend interval(Tp-Te)and left ventricular ejection fraction(LVEF)in elderly patients with heart failure with reduced ejection fraction(HFrEF),and to evaluate their combined predictive value for malignant arrhythmia(MA)occurrence.Methods Clinical data of 127 elderly patients with HFrEF admitted to Xinyu Yuanhe Hospital from January 2022 to September 2024 were retrospectively ana-lyzed,and they were divided into MA group(45 patients)and non-MA group(82 patients)according to whether MA occurred within 1 month after diagnosis.The correlation between electrocardiographic index Tp-Te interval and LVEF were analyzed by Pearson correlation,and the influencing factors were analyzed by logistic regression.Results Pearson correla-tion analysis revealed that the electrocardiographic indices—Tp-Te interval,corrected Tp-Te interval(Tp-Tec),and Tp-Te interval dispersion(Tp-Ted)—were all positively correlated with the ejection fraction parameters,namely left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),and left ventricular mass index(LVMI)(r=0.845-0.990,P＜0.01).In contrast,LVEF was negatively correlated with all the aforementioned indices(r=-0.891 to-0.984,P＜0.01).Multivariable regression analysis demonstrated that Tp-Te(β=0.062,OR=1.064,95％CI:1.008-1.123),Tp-Tec(β=0.079,OR=1.082,95％CI:1.017-1.153),LVEF(β=-0.577,OR=0.562,95％CI:0.363-0.868),LVEDD(β=0.364,OR=1.440,95％CI:1.043-1.987),and LVMI(β=0.091,OR=1.095,95％CI:1.024-1.170)were all identified as independent influencing factors for the occurrence of MA in patients with HFrEF(P＜0.05).Draw the receiver operating characteristic(ROC)curve using SPSS software,the Yoden index was 0.911,the AUC was 0.986,95％CI:0.967-1.000,with a sensitivity of 0.911 and a specificity of 1.000.Conclusion Based on the elderly patients confirmed to have HFrEF,electri-cal indicators Tp-Te,Tp-Tec,Tp-Ted and ejection fraction indicators LVEDD,LVESD and LVMI were all positively correlated,and significantly negatively correlated with LVEF.Furthermore,the Tp-Te interval and ejection fraction parameters can be used for joint prediction of MA occurrence,demonstrating good predictive performance.These findings can provide valuable reference for clinical practice.关键词
