动力位MRI在无骨折脱位型颈脊髓损伤诊断及治疗中的应用
The application of diagnosis and treatment of kinematic MRI in cervical spinal cord injury without fracture and dislocation

Abstract
Objective To explore the application value of kinematic MRI in the diagnosis and treatment of cervical spinal cord injury without fracture and dislocation.Methods In addition to the conventional neutral position MRI examination,hyperextension and hyperflexion position MRI examinations were performed on 21 patients with cervical spinal cord injuries without fracture and dislocation.The degree of spinal stenosis was classified by the Muhle scale on the median sagittal T2 WI sequence of kinematic MRI,and the range of spinal cord injury was measured on the me-dian sagittal T2 WI sequence of kinematic MRI,further,the degree of spinal cord injury was graded.Results Com-pared with the neutral position MRI,the number of spinal stenosis segments in the hyperextension position MRI was increased,the degree of spinal canal stenosis at the affected segment was partially aggravated,and the degree of spinal cord injury was partially aggravated.After a comprehensive assessment of kinematic MRI,the number of cervical ver-tebrae with surgical indicators was increased from 18 segments to 22 segments on the neutral position MRI,among them,one segment changed from non-surgical treatment to surgical treatment due to changes in spinal cord signals in the hyperextension position;three segments suffered from the spinal canal stenosis and spinal cord injuries due to the new increasing anterior compression.Conclusions Kinematic MRI can detect potential spinal cord compression and spinal cord injury segments,clarify the degree of injury,and further reveal the kinematic changes of the anterior and posterior structures of the cervical spinal cord.It serves as important supplement to conventional neutral position MRI,and provides important reference value for the precise diagnosis and selection of treatment plans for patients with cer-vical spinal cord injury without fracture and dislocation.关键词
