两种颈椎前路手术治疗颈椎病的疗效比较
Comparison effect of surgery treatment of the cervical spondylosis via two types of anterior cervical approaches

Abstract
Objective To compare the efficacy of double-segment anterior cervical discectomy and fusion(ACDF)and single-segment anterior cervical corpectomy and fusion(ACCF)in the treatment of cervical spondylosis.Methods Forty-four patients with cervical spondylosis who underwent anterior cervical surgery were divided into the ACCF group(treated with single-segment ACCF,26 cases)and the ACDF group(treated with double-segment ACDF,18 cases)according to different surgical methods.The operation time,intraoperative blood loss,C2～7 Cobb angle,Cobb angle of the surgical segment,lower cervical spinal lordosis angle(CSA),C2～7 sagittal vertical axis(C2～7 SVA),neck disability index(NDI),and anterior-posterior margin height of the surgical segment were recorded.Results All pa-tients were followed up for 1～3 years.There were no statistical differences in the operation time and intraoperative blood loss between the two groups(P>0.05).The C2～7 Cobb angle,the Cobb angle of the surgical segment,CSA,C2～7 SVA,NDI,and the height of the anterior-posterior margin of the surgical segment in both groups were improved at 1 week after the operation and at the last follow-up,compared with those before the operation(P<0.05).One week after the operation and at the last follow-up,the ACDF group was all superior to the ACCF group in the terms of the C2～7 Cobb angle,CSA,C2～7 SVA,and the anterior-posterior margin height of the surgical segment(P<0.05).There were no statistical differences in the Cobb angle and NDI of the surgical segments between the two groups(P>0.05).Conclusions Both two-segment ACDF and single-segment ACCF can improve the sagittal cervical balance and cervical function of cervical spondylosis patients after surgery.Two-segment ACDF has more advantages in the sagittal cervical balance after surgery.关键词
