Abstract
Objective To compare the efficacy of unilateral and bilateral puncture percutaneous kyphoplasty(PKP)in the treatment of osteoporotic vertebral compression fracture(OVCF).Methods The 216 patients with OVCFs were divided into a unilateral puncture group(The 63 patients were treated with unilateral puncture PKP)and a bilat-eral puncture group(The 153 patients were treated with bilateral puncture PKP)according to different puncture methods.The following data were compared between the two groups,including the intraoperative fluoroscopy frequen-cy,intraoperative blood loss,operation duration,bone cement injection volume,postoperative first ambulation time,postoperative complication occurrence,and radiographic observation indicators.The pain VAS was used to evaluate the pain condition,and the ODI score was used to assess the functional improvement.Results All patients were fol-lowed up for 3～6 months.The frequency of intraoperative fluoroscopy,intraoperative blood loss,bone cement injec-tion volume,and operation time in the unilateral puncture group were all less(shorter)than those in the bilateral puncture group(P<0.05).The postoperative first ambulation time had no statistical difference between two groups(P>0.05).There were no statistical differences in the postoperative immediate central height,anterior edge height of the injured vertebra,and Cobb angle between the two groups(P>0.05).There were no statistical differences in the pain VAS and ODI score between the two groups at 1 d after surgery(P>0.05).No complications such as pulmo-nary embolism,nerve injury,or bone cement toxic reaction occurred in either group after surgery.There was no statisti-cal difference in the postoperative bone cement leakage rate between the two groups(P>0.05).Conclusions The unilateral and bilateral puncture PKP treatments for OVCF show similar effects in terms of postoperative pain relief,functional recovery,and bone cement leakage.However,the unilateral puncture is more suitable for elderly patients with poor physical conditions or those who cannot tolerate long surgeries due to its advantages such as simple opera-tion,less trauma,and shorter operation time.关键词
