两种椎体强化术治疗胸腰段骨质疏松性椎体压缩骨折的疗效比较

Abstract
Objective To compare the effect of percutanous kyphoplasty(PKP)and percutaneous vertebroplasty(PVP)in the treatment of thoracolumbar osteoporotic vertebral compression fracture(OVCF).Methods The 86 pa-tients with thoracolumbar OVCFs were divided into PKP group(The 43 cases were treated with PKP)and PVP group(The 43 cases were treated with PVP),according to different surgical methods.The surgical time,incidence of compli-cations,and imaging observation indicators were compared between two groups.Pain VAS was used to evaluate the pain intensity in the lumbosacral and gluteal regions.ODI and Bathel index were used to evaluate the functional recov-ery.Results All patients were followed up for 6～31(18.3±39.1)months.There was no statistical difference in surgical time between the two groups(P>0.05).Both groups did not experience complications such as nerve or vas-cular damage or pulmonary embolism during the surgery;the leakage rate of bone cement in the PKP group was lower than that in the PVP group(P<0.05).There were no statistical differences in ODI and Bathel index between the two groups on the second day after surgery and at the last follow-up(P>0.05).At the last follow-up,the pain VAS for lumbosacral and buttock hip pain was lower in the PKP group than that in the PVP group(P<0.05).On the second day after surgery and at the last follow-up,the anterior edge height,middle height,and Cobb angle of the injured verte-brae in the PKP group were superior to those in the PVP group(P<0.05).Conclusions PKP and PVP are both ef-fective methods for treating thoracolumbar OVCF,while PKP has more advantages in reducing bone cement leakage,restoring vertebral height,improving Cobb angle,and alleviating pain in the lumbosacral and buttock region.关键词
