Application of medication literacy promotion program based on the theory of behavior change wheel in elderly disabled patients with diabetes mellitus and their caregivers

Abstract
Objective To explore the application effect of the medication literacy promotion program based on the theory of behavior change wheel(BCW)in elderly disabled patients with diabetes mellitus and their caregivers.Methods Thirty-eight elderly disabled patients with diabetes mellitus admitted to Hai'an People's Hospital in Jiangsu Province from July 2024 to October 2024 were selected as a control group,and thirty-eight patients admitted to our hospital from December 2024 to February 2025 were selected as an observation group.The control group received routine nursing,while the observation group received a medication literacy promotion program based on the theory of BCW.Both groups received continuous intervention until the patients were discharged,with a follow-up for 3 months after discharge.The medication literacy score,the Morisky Medication Adherence Scale-8(MMAS-8)score,and Medication Discrepancy Tool(MDT)score,and unplanned readmission status were compared between the two groups.Results At 1 and 3 months after discharge,the medication literacy scores of the observation group and their caregivers were higher than those of the control group.The MMAS-8 scores of the observation group were higher than those of the control group,and the MDT scores of the observation group were lower than those of the control group(P<0.05).Both groups were better compared with at admission(P<0.05).There was no statistically significant difference in the unplanned readmission rate between the two groups within 3 months after discharge(P>0.05).Conclusion The medication literacy promotion program based on the theory of BCW is conducive to improving the medication literacy level of elderly disabled patients with diabetes mellitus and their caregivers,and can improve medication compliance and reduce the risk of medication discrepancy.关键词
