Professor ZHANG Qiwen's Experience in Treating Pediatric Allergic Rhinitis Based on the Theory of Collateral Disease

Abstract
Professor ZHANG Qiwen clinically applies the theory of collateral disease,based on the prin-ciple that"prolonged illness involves the collaterals,with stagnation of both qi and blood,"to treat pediatric al-lergic rhinitis(AR)with notable therapeutic outcomes.Professor ZHANG posits that disorders of any of the five zang organs can lead to the onset of AR.Its pathogenesis is characterized by a mixture of deficiency and excess,with the key mechanism being obstruction of the nasal collaterals,while insufficiency of lung,spleen,and kid-ney qi underlies the recurrence and persistence of the condition.In the acute stage of pediatric AR,the mani-festation is primarily excess.Treatment should focus on eliminating pathogenic factors and unblocking the col-laterals.Modifications are made according to the pattern:for wind-heat exterior pattern,Yinqiao San(银翘散)is used;for wind-cold exterior pattern,Guizhi Tang(桂枝汤)or Mahuang Tang(麻黄汤)is applied;for Yang-ming internal heat pattern,Baihu Tang(白虎汤)is prescribed;for liver-gallbladder damp-heat pattern,Long-dan Xiegan Tang(龙胆泻肝汤)is administered;and for spleen-stomach damp-heat pattern,Wendan Tang(温胆汤)is utilized.In the intermediate stage,deficiency is the primary aspect.Treatment should aim at supple-menting and nourishing to unblock the collaterals.For cases with prolonged disease tending toward lung-spleen deficiency,modified Yupingfeng San(玉屏风散)combined with Sijunzi Tang(四君子汤)is used;for cases tending toward spleen-kidney deficiency pattern,modified Zhenwu Tang(真武汤)is employed.In the late stage,phlegm and stasis dominate.Treatment should focus on removing stasis and resolving phlegm to unblock the collaterals,using a modified version of Professor ZHANG's empirical formula,Yanmen Suotao Wan(咽门缩桃丸).Given the complex etiology and pathogenesis of pediatric AR,clinical practice should based on pat-tern differentiation and treatment,consistently incorporate the method of unblocking the collaterals and dissi-pating nodules throughout the entire treatment process.One case example is attached for verification.关键词
