Abstract
Objective:To investigate the relationship between quantitative parameters of 3.0T magnetic resonance imaging(MRI)and diffusion tensor imaging(DTI)of the head and neurological damage in acute cerebral infarction(ACI),and to evaluate their value in predicting hemorrhagic transformation(HT)after thrombolysis.Methods:Eighty-four patients with ACI from September 2022 to September 2024 were selected,and all of them were examined by DTI quantitative parameters[fractional anisotropy(FA),mean diffusivity(MD),volume ratio anisotropy(VRA)and attenuation index(Exat)].The NIHSS score was used to assess the degree of neurological damage before treatment,and patients were divided into mild,moderate,and severe groups based on their scores.The DTI quantitative parameters and NIHSS scores of the three groups were compared,and the relationship between DTI quantitative parameters and NIHSS scores was analyzed.Meanwhile,patients were divided into HT and non-HT groups based on head CT results within 2 days after thrombolysis.The baseline characteristics and DTI quantitative parameters of the two groups were compared,and the relationship between DTI quantitative parameters and the occurrence of HT after thrombolysis in ACI patients was analyzed.Results:The FA,MD,and VRA values in the infarcted area were lowest in the severe group,while the Exat values and NIHSS score were highest(P<0.05).FA,MD,and VRA values were negatively correlated with NIHSS scores,while the Exat value was positively correlated with NIHSS scores.Compared with the non-HT group,the HT group had higher NIHSS scores,lower FA and VRA values,and a longer time from onset to thrombolysis(P<0.05).Binary Logistic regression analysis showed that FA and VRA values were negatively associated with the occurrence of HT after thrombolysis in ACI patients(P<0.05).ROC curve analysis showed that the AUC for FA,VRA values,and their combination in predicting the occurrence of HT after thrombolysis in ACI patients were 0.793,0.791,and 0.802,respectively,with the best combined predictive effect.Conclusion:3.0T MRI and DTI quantitative parameter examinations of the head can effectively assess the degree of neurological damage in ACI patients,and the combination of FA and VRA values is effective in predicting the occurrence of HT after intravenous thrombolysis.关键词
