基于多microRNA表达水平构建的卒中后认知障碍风险预测模型的效果分析
Effect analysis of risk prediction model of post-stroke cognitive impairment based on multi-microRNA expression level

Abstract
Objective To investigate the predictive effect of a multi-microRNA(miRNA)expression level model on the risk of post-stroke cognitive impairment(PSCI).Methods A total of 298 patients diagnosed with stroke in our hospital from January 1,2021 to December 31,2023 were selected as the study subjects.All patients were divided into PSCI group and non-PSCI(PSCN)group according to MMSE scale results.Clinical data of patients were collected,and the contents of miR-21,miR-132,miR-200b,miR-8485 and miR-449b in peripheral blood were detected by qRT-PCR.Logistic regression was used to analyze the influencing factors of PSCI occurrence,and finally the prediction model of PSCI occurrence was built based on the microRNA expression level.Results Age and the ratio of previous stroke history in PSCI group was significantly higher than those in PSCN group(all P<0.05).The ratio of homocysteine,cortical infarct location and number of lesions ≤2 in PSCI group were significantly higher than those in PSCN group(all P<0.05).miR-21,miR-132,miR-200b,miR-8485 and miR-449b in PSCI group were significantly lower than those in PSCN group(all P<0.05).homocysteine,stroke history and supratentorial infarction were independent risk factors for PSCI.The number of lesions(1),miR-21,miR-132,miR-200b,miR-8485 and miR-449b were independent protective factors affecting the occurrence of PSCI(all P<0.05).The C-index of the nomogram model for predicting the occurrence of PSCI was 0.983(95%CI:0.972-1.000),and the differentiation was good.Conclusion The PSCI risk prediction model based on the expression levels of miR-21,miR-132,miR-200b,miR-8485 and miR-449b has certain clinical application value,it is also necessary to expand the collection of a large number of clinical cases to improve the construction of the model.关键词
