Exploration of the pathogenesis and prevention of recurrent urinary tract infections based on the theory of vital qi and pathogenic factors

Abstract
Uropathogenic Escherichia coli(UPEC)is the principal pathogen behind recurrent urinary tract infections(RUTI),a challenging condition to eradicate in clinic.In traditional Chinese medicine(TCM),UPEC is classified under the concept of"Xie"(pathogenic factors).Contemporary studies have illustrated that UPEC in RUTI are characterized as its adhesion ability,biofilm formation and secretion of virulence factors.These pathogenic activities are more likely to take place when the hosts' defensive function,or body resistance,is compromised,facilitating UPEC's evasion of the immune system and inducing the RUTI onset.This paper discusses the TCM pathogenesis underlying UPEC-induced RUTI through the lens of the theory of vital qi and pathogenic factors,providing insights into the disease's pathogenesis characterized by the interplay between UPEC as the pathogen and the hosts' body resistance.It reveals that the essential pathogenesis of RUTI is the syndrome of deficient vital qi and pathogen accumulation,with the latter as its manifestation,which causes damages in the bladder,and with the former as its root cause,which is primarily attributed to the spleen and kidney.Accordingly,the treatment principles of strengthening vital qi to eliminate pathogenic factors in RUTI are explored,emphasizing the simultaneous application of strategies to weaken pathogenic factors and nurture vital qi.The idea of addressing persistent pathogens while reinforcing spleen and kidney functions to invigorate vital qi and prevent its recurrence forms a theoretical and practical basis for the clinical management of RUTI.关键词
