颞下颌关节盘前移位患者翼外肌附着及纹理特征分析
Evaluation of lateral pterygoid muscle attachment and texture in anterior disc displacement of the temporoman-dibular joint

Abstract
Objective:To analyze the differences in lateral pterygoid muscle(LPM)attachment types and texture fea-tures in patients with anterior disc displacement(ADD)of the temporomandibular joint.Methods:A total of 72 subjects(144 sides),comprising 57 females and 15 males with a mean age of(28.21±10.45)years,were enrolled.Based on disc position,the subjects were divided into three groups:normal articular disc position(NADP)group(41 sides),anterior disc displace-ment with reduction(ADD WR)group(58 sides),and anterior disc displacement without reduction(ADDWoR)group(45 sides).The LPM attachment types and texture parameters,including angular second moment,contrast,inverse difference mo-ment,and entropy,were compared among the groups.The diagnostic performance of each parameter was evaluated using re-ceiver operating characteristic(ROC)curve analysis,and the area under the curve(AUC)values.Results:Regarding LPM at-tachment types,a statistically significant difference was observed among the three groups(x2=22.36,P=0.004).No signifi-cant differences were found between the NADP and ADDWR groups or between the ADDWR and ADDWoR groups(P>0.0167),whereas a significant difference existed between the NADP and ADDWoR groups(P<0.0167).For texture parame-ters,the differences in angular second moment,contrast,inverse difference moment,and entropy of the LPM among the three groups were statistically significant(P<0.05).No significant differences in angular second moment,contrast,inverse differ-ence moment,or entropy were observed between the NADP and ADDWR groups(P>0.0167).Compared to the NADP and ADDWR groups,the ADDWoR group showed significantly increased angular second moment and inverse difference moment,and significantly decreased contrast and entropy(P<0.0167).ROC analysis confirmed that the texture parameter entropy demonstrated good diagnostic efficacy in distinguishing the NADP from the ADDWoR(AUC=0.74),with a cutoff value of 8.040,sensitivity of 57.78%,and specificity of 78.05%.In contrast,angular second moment,contrast,and inverse difference moment showed poor efficacy(AUC<0.70)in discriminating the ADDWoR from either the NADP or ADDWR,and entropy al-so showed poor efficacy(AUC<0.70)in discriminating ADDWR from ADDWoR.Conclusion:In summary,compared to the NADP and ADDWR groups,patients with ADDWoR exhibit significant differences in LPM attachment types and texture pa-rameters.Entropy is a potential marker for the auxiliary diagnosis of ADDWoR.关键词
