ICU肠内营养中断管理的审查指标构建及障碍因素分析
Development of clinical audit criteria and analysis of barrier for prevention and management of enteral nutrition interruption in ICU patients

Abstract
Objective To develop evidence-based audit criteria for the prevention and management of enteral nutrition(EN)interrup-tions in ICU patients,conduct a baseline clinical audit,and identify barriers and facilitators to inform evidence translation strategies.Methods This study was guided by the Clinical Evidence Implementation Model from the Fudan University JBI Centre for Evidence-Based Nursing.An evidence-based practice team was established to identify clinical problems,retrieve and appraise evidence,and synthesize the best available evidence.Audit criteria and methods were established,and a baseline audit was conducted.Barriers and facilitators were analyzed using the integrated-Promoting Action on Research Implementation in Health Services(i-PARIHS)framework across innovation,recipients,and context domains.Corresponding change strategies were developed.Results A total of 23 pieces of best evidence were synthesized into 20 audit criteria.Baseline audit findings showed that compliance with 14 of these criteria was be-low 80%,and among these,two criteria had a compliance rate of 0%.Major barriers included non-specific and inaccessible evidence(innovation level),increased nursing workload and insufficient knowledge and skills(recipient level),and lack of multidisciplinary com-munication mechanisms and specific tools for EN interruption management(context level).Conclusion There is a substantial gap be-tween current evidence and clinical practice regarding prevention and management of EN interruption in ICU.Evidence-based project leaders should proactively address barriers and leverage facilitators to promote the successful clinical translation of evidence.关键词
