胸腔镜手术患者急性疼痛变化轨迹的潜在类别及影响因素分析
Potential categories and influencing factors of acute pain trajectories in patients undergoing thoracoscopy

Abstract
Objective To explore the potential categories and influencing factors of acute pain trajectories in patients undergoing tho-racoscopy.Methods Convenient sampling was used to select patients with pulmonary nodules who were scheduled for elective thora-coscopic surgery from May to August 2024 as investigation objects,and patients'demographic,disease and surgical data were collect-ed.The pain severity was assessed by the Numeric Rating Scale from the 1st to 6th day after surgery,and the potential category of pain change trajectory was identified by the latent growth mixture model,and its influencing factors were analyzed by multivariate logistic re-gression.Results Totally 321 patients with thoracoscopy were classified into four potential categories:mild persistent pain group(52.0%),mild to moderate pain relief group(17.1%),pain exacerbation and then relief group(15.0%)and moderate pain relief group(15.9%).Multivariate logistic regression analysis showed that sex,operation type,use of intercostal nerve block,use of patient-con-trolled analgesia,preoperative anxiety and number of thoracoscopic holes were independent predictors of acute pain trajectory catego-ries(P<0.05).Conclusion Patients with thoracoscopy show different acute pain trajectories.It is imperative to direct clinical vigilance and optimize interventions for patients with the following characteristics:female sex,performance of lobectomy,absence of intercostal nerve block,non-utilization of patient-controlled analgesia,presence of preoperative anxiety,or use of a three-port surgical approach.关键词
