Abstract

Objective To explore the health economic evaluation of a combined model for active screening for multidrug-resistant organisms and the construction of stratified prevention and control nursing in intensive care unit(ICU)in China,and to address the is-sue of emphasizing uniform prevention over stratified approaches.Methods A pre-post comparative design was employed,enrolling 431 inpatients in the ICU from July 1,2023,to September 30,2024.Participants were divided into a control group and an intervention group.The control group received standardized bundled prevention measures,while the intervention group active screening combined with stratified prevention and nursing interventions.hospital-acquired infection incidence of multidrug-resistant bacteria,length of stay,and economic burden were compared between the two groups.Cost-effectiveness analysis and incremental cost-effectiveness ratio were used to evaluate control efficacy.Results The intervention group demonstrated significantly lower rate of hospital-acquired in-fection and multidrug-resistant organism infection compared to the control group.Median total length of stay was reduced by 9 days,and the ICU stay for patients with multidrug-resistant organism infection was shortened by 26 days(P<0.05).The intervention group in-curred an additional cost of¥619 per case compared to the control group.However,direct medical cost,direct non-medical cost,indi-rect economic burden,and disease-related economic burden were lower in the intervention group(P<0.05).The calculated ICER was ¥8,365 per case.Conclusion Nursing-led stratified prevention and control measures for multidrug-resistant organisms in the ICU significantly reduce hospital infection rate and economic burden while demonstrating favorable cost-effectiveness,providing evidence for its implementation in high-risk wards.
