强骨止痛方对去卵巢大鼠膝关节软骨下骨的骨保护效应研究
Protective Effect of Qianggu Zhitong Formula on Knee Subchondral Bone in Ovariectomized Rats

Abstract
Objective To investigate the effects of Qianggu Zhitong Formula on serum markers,biomechanics,bone microstructure,and biomechanical parameters of tibial plateau subchondral osteoporosis in the knee joint of ovariectomized osteoporotic rats.Methods Forty SPF female SD rats were divided into a sham operation group,an osteoporosis model group,TCM group,and an Western medicine group by the random number table method,with 10 rats in each group.Adipose tissue of the same size around the ovaries was removed in the sham operation group,while bilateral ovariectomy was conducted in the other three groups to establish osteoporosis models.After successful modeling at 12 weeks,medicine intervention was initiated.The traditional Chinese medicine(TCM)group received Qianggu Zhitong Formula at a crude medicine dose of 12.96 g(kg·d),and the Western medicine group received alendronate sodium at a dose of 6.3 mg/kg/week;the other groups were gavaged with an equal volume of normal saline.After intervention for 12 weeks,samples were collected for detection.Enzyme-linked immunosorbent assay(ELISA)was used to detect the serum concentrations of bone metabolism,estrogen,and inflammation-related factors:total procollagen type 1 N-terminal propeptide(PINP),cross-linked C-telopeptide of type I collagen(CTX-1),estradiol(E2),interleukin-6(IL-6),and tumor necrosis factor-alpha(TNF-α).Micro-CT was employed to analyze the subchondral bone microstructure,trabecular,and mineral parameters of the tibial plateau cartilage.Hematoxylin-eosin(HE)staining and safranin O-fast green staining were performed on the bone tissue sections of the tibial plateau to observe the structure of the subchondral bone and cartilage of the tibial plateau.Finally,the indentation experiment was conducted on the tibial plateau to measure the biomechanical properties of the subchondral bone of the tibial plateau.Results Serum PINP,CTX-1,and E2 levels in the TCM group and the Western medicine group were significantly higher than those in the model group(P<0.05),but there was no significant difference between the TCM group and the Western medicine group(P>0.05).Serum IL-6 and TNF-α levels in the TCM group and the Western medicine group were significantly lower than those in the model group,while there was no statistically significant difference between the two groups(P>0.05).Micro-CT results showed that the bone mineral density(BMD),trabecular thickness(Tb.Th)and trabecular number(Tb.N)of the tibial subchondral bone in the TCM group and the Western medicine group were significantly higher than those in the model group(P<0.05),while the trabecular separation(Tb.Sp),and structure model index(SMI)were significantly lower than those in the model group(P<0.05),with no significant difference between the TCM group and the Western medicine group(P>0.05).In HE staining of subchondral bone tissue sections of the tibial plateau,Tb.Th and Tb.N in the Qianggu Zhitong Formula group and the Western medicine group were significantly higher than those in the model group;in safranin O-fast green staining,articular cartilage damage degree,chondrocyte number and arrangement,as well as cartilage surface smoothness in the two groups were significantly advantageous over those in the model group.The biomechanical properties,such as displacement,load,area,and slope of the subchondral bone of the tibial plateau in the Qianggu Zhitong Formula group and the Western medicine group were significantly better than those in the model group(P<0.01),but there was no significant difference compared with the Western medicine group(P>0.05).Conclusion Qianggu Zhitong Formula can effectively regulate serum bone metabolism,estrogen,and inflammatory factors in ovariectomized rats.The Decoction improves the bone microstructure and trabecular bone parameters of the subchondral bone of the tibial plateau and inhibits bone loss,enhancing the mechanical properties of the spongy bone under the knee joint cartilage.关键词
