针灸治疗帕金森病自主神经功能障碍疗效的Meta分析
Meta Analysis on the Efficacy of Acupuncture and Moxibustion for Autonomic Nerve Dysfunction in Parkinson's Disease

Abstract
Objective To systematically evaluate the clinical efficacy of acupuncture and moxibustion for autonomic nerve dysfunction in Parkinson's Disease(PD).Methods A computerized search was performed in China National Knowledge Infrastructure(CNKI),Wangfang Data,China Science and Technology Journal Database(VIP),and SinoMed,The Cochrane Library,PubMed,Embase,and Web of Science to select the randomized clinical trials(RCTs)of acupuncture and moxibustion for autonomic nerve dysfunction in PD from the database establishment to April 2024.Outcome indicators included the total clinical effective rate for symptoms related to autonomic nerve dysfunction in PD,standardized swallowing assessment(SSA)scores,video fluoroscopy swallow study(VFSS)scores,patient assessment of constipation quality of life(PAC-QOL)scores,chronic constipation scale(CSS)scores,the incidence of nausea,average voided volume,nocturnal polyuria,average 24-hour urinary incontinence,the incidence of palpitations,cardiovascular symptom scores,and the incidence of postural hypotension.Statistical analysis was performed using RevMan 5.4 and Stata 12.0.Results A total of 31 papers were included,involving 2396 patients.Meta analysis revealed that compared with the control group,the experimental group demonstrated a significant advantage in increasing total clinical effective rate of symptoms related to autonomic nerve dysfunction in PD[RR=1.23,95%CI=(1.16,1.31),P<0.000 01],VFSS scores[MD=1.48,95%CI=(1.16,1.81),P<0.000 01],and average voided volume[MD=73.25,95%CI=(45.58,100.92),P<0.000 01],reducing SSA scores[MD=-2.93,95%CI=(-3.76,-2.10),P<0.000 01],PAC-QOL scores[MD=-11.07,95%CI=(-15.15,-6.98),P<0.000 01],CSS scores[MD=-1.91,95%CI=(-2.83,-0.99),P<0.000 1],and profuse sweating symptom score[MD=-1.47,95%CI=(-1.69,-1.24),P<0.000 01],as well as improving nausea symptoms[RR=0.45,95%CI=(0.34,0.60),P<0.000 01]and palpitations[RR=0.63,95%CI=(0.45,0.88),P<0.007].No significant statistical differences were observed between the two groups in nocturnal polyuria,24-hour average urinary incontinence,cardiovascular symptom scores,or the incidence of postural hypotension(P>0.05).Conclusion Acupuncture and moxibustion treatment effectively increase the overall effective rate for symptoms related to autonomic nerve dysfunction in PD and improve dysphagia,constipation,nausea,oliguria,palpitations,and profuse sweating in PD patients.However,it exhibits no significant advantage over conventional therapy in improving postural hypotension,nocturnal polyuria,and urinary incontinence.These findings require further validation through more high-quality RCTs.关键词
