Abstract
Objective:To compare the differences in antimicrobial prescription review outcomes among the antimicrobial stewardship(AMS)team,clinical pharmacists,and orthopedic clinicians,and to explore the role and value of the AMS model in prescription review.Methods:Using stratified sampling,200 antimicrobial prescriptions from the Orthopedics Department of Nanfang Hospital,Southern Medical University,between October 2023 and January 2024 were extracted from the Hospital Information System.The AMS team,clinical pharmacists,and orthopedic clinicians independently reviewed the prescriptions.The review outcomes from the three groups were compared across dimensions including overall rationality,indication for use,correct drug selection,indication for combination therapy,appropriateness of dosage and administration,determination of medication purpose,and diagnostic criteria standardization for therapeutic use.Statistical analysis was performed using Cochran's Q test,McNemar test,and Kendall's W test.Results:Regarding overall rationality,the rate of appropriate prescriptions in the AMS group(70.5%)was lower than that in the pharmacy group and the clinical group(both 82.0%,P<0.001).Statistically significant differences were observed among the three groups in the indications for combination therapy,determination of medication purpose,and diagnostic criteria standardization for therapeutic use(P<0.05).In contrast,no significant differences were found among the groups in indication for use,correct drug selection,and appropriateness of dosage and administration(P>0.05),with a high level of agreement(Kendall's W coefficients all>0.5,P<0.001).Conclusion:Prescription review under the AMS model demonstrates a more stringent and comprehensive perspective.It particularly compensates for the limitations of single-specialty reviews in complex medication decision-making,thereby contributing to enhanced quality and depth of prescription review and promoting the rational clinical use of antimicrobials.关键词
