The Incidence,Risk Factors and Clinical Significance of Pneumothorax and Pulmonary Hemorrhage After CT-guided Percutaneous Transthoracic Lung Biopsy

Abstract
Objective To investigate the incidence,clinical significance and risk factors of pneumothorax(PTX)and pulmonary hemorrhage(PH)after computed tomography(CT)-guided percutaneous transthoracic lung biopsy(PTLB).Methods A total of 75 patients who underwent CT-guided percutaneous transthoracic lung biopsy in the Department of Respiratory and Critical Care Medicine,the Second Affiliated Hospital of Wannan Medical College from June 2022 to December 2024 were selected as the research objects.The demographic data,combined underlying diseases,lesion characteristics,lesion location,distance from the lesion to the pleura,puncture depth and pathological results were collected.The incidence and types of pneumothorax and pulmonary hemorrhage were analyzed.Subgroup analysis of pulmonary hemorrhage and pneumothorax was performed to compare the differences between the two groups.The influencing factors of pneumothorax and pulmonary hemorrhage after PTLB were analyzed.Results The incidence of pneumothorax after PTLB was 25.33%,and the incidence of postoperative pulmonary hemorrhage was 36.00%.Subgroup analysis of pneumothorax showed that there were significant differences in emphysema and distance from the lesion to the pleura between the two groups(P＜0.05).Subgroup analysis of pulmonary hemorrhage showed that there were significant differences in BMI and diabetes mellitus between the two groups(P＜0.05).Further analysis showed that emphysema was an independent risk factor for pneumothorax(OR=5.057).Conclusion CT-guided PTLB is a safe and effective method for lung biopsy.Emphysema may be an important cause of pneumothorax after operation.关键词
