Abstract
Objective:To explore the effect of laparoscopic totally extraperitoneal hernia repair(TEP)combined with transurethral thulium laser enucleation of prostate(ThuLEP)in the same period for patients with inguinal hernia complicated with benign prostatic hyperplasia(BPH),and to provide an economical and safe treatment plan for clinical physicians.Method:A retrospective collection was conducted of 748 patients with inguinal hernia complicated with BPH who were admitted to the Department of Hernia and Abdominal Wall Surgery of the First Hospital of Shanxi Medical University from April 2020 to April 2024.According to different surgical methods,they were divided into the concurrent group(TEP combined with ThuLEP in the same period)and the non-concurrent group[TEP was performed first,and then the patient was re-admitted for ThuLEP after a good recovery(usually 3 months after the surgery)].There were 396 cases in the concurrent group and 352 cases in the non-concurrent group.The operation-related indexes,urodynamic indexes,international prostate symptom score(IPSS),complications and hernia recurrence rate of the two groups were compared.Result:The postoperative bladder irrigation time and total length of hospital stay of the concurrent group were shorter than those of the non-concurrent group,and the total hospitalization cost was lower than that of the non-concurrent group(P<0.05).Three months after the operation,the residual urine volume and maximum detrusor pressure of both groups were significantly lower than those before the operation,and the maximum urine flow rate and bladder compliance were significantly higher than those before the operation,with statistically significant differences(P<0.05).Three months after the operation,the maximum urine flow rate of the concurrent group was higher than that of the non-concurrent group(P<0.05).The IPSS of both groups three months after the operation were lower than those before the operation,with statistically significant differences(P<0.05).The complication rate of the concurrent group was lower than that of the non-concurrent group,with a statistically significant difference(P<0.05);the recurrence rate of hernia in the concurrent group was lower than that in the non-concurrent group,but the difference was not statistically significant(P>0.05).Conclusion:TEP combined with ThuLEP in the same period treatment of inguinal hernia complicated with BPH can effectively reduce the length of hospital stay and hospitalization cost,promote postoperative recovery,improve urodynamic indexes,and reduce the complication rate.关键词
