Abstract

Objective:To investigate the application of combined detection of cardiac auscultation and portably integrated device for blood oxygen in screening congenital heart disease(CHD)on the Tibetan Plateau.Methods:A retrospective analysis was conducted on the clinical data of 3000 children aged 0-14 years in Qinghai Province from January 2023 to December 2024.According to different detection methods,they were divided into blood oxygen group,cardiac auscultation group and combination group with integrated device,with 1000 patients in each group.The blood oxygen group underwent fingertip measurement for blood oxygen saturation by using portable pulse oximeter,the Heart Auscultation Group underwent cardiac auscultation using the portable integrated heart auscultation and blood oxygen detection device.The Combined Detection Group received both tests simultaneously.Using cardiac ultrasonography as the"gold standard,"indicators such as screening positive rate,confirmation rate,sensitivity,specificity,and likelihood ratios were compared among the three groups.Results:The positive rate in the combined detection group was 11.00%(110/1000),and the confirmation rate was 4.40%(44/1000),both of which were significantly higher than those in the blood oxygen group(8.40%[84/1000]and 2.70%[27/1000]),with statistically significant differences(x2=3.859,4.220,P<0.05).These values were also higher than those in the cardiac auscultation group(8.10%[81/1000]and 2.50%[25/1000]),and the differences were statistically significant(x2=4.868,5.419,P<0.05).The diagnostic sensitivity of the combined detection group was 88.00%,which was significantly higher than that of the blood oxygen group(60.00%)and the cardiac auscultation group(50.00%).The difference between the three groups was statistically significant(x2=42.36,P<0.05).The diagnostic specificity of the combined detection group was 93.05%,the blood oxygen group was 93.93%,and the cardiac auscultation group was 94.11%.There was no significant difference between the three groups(P=0.591).Conclusion:Combined detection using the portable integrated heart auscultation and blood oxygen device demonstrates significant advantages in screening for CHD on the Tibetan Plateau.It can substantially improve the positive rate,confirmation rate,and sensitivity while maintaining high specificity,providing an efficient and reliable method for early disease screening.
