Abstract
Systemic lupus erythematosus(SLE)is a systemic autoimmune disease characterized by frequent complications such as hair loss,which exhibits high incidence and serves as one of the significant indicators of disease activity.The etiology remains incompletely understood,and Western medicine primarily attributes this alopecia to immune complex deposition-induced hair follicle damage.Current Western medical treatments for systemic lupus alopecia primarily target SLE itself,with slow efficacy in alleviating hair loss symptoms,adversely affecting patients'physiological and psychological well-being.Traditional Chinese medicine(TCM)has shown remarkable effectiveness in treating SLE and its comorbidities with minimal adverse reactions and strong individualization,and it is necessary to explore the pathogenesis and therapeutic approaches for systemic lupus alopecia based on diverse TCM theories.TCM theory of"deficient Qi(气)stagnation"is widely applied in fields such as encephalopathy,gynecological disorders,and rheumatic immune diseases.In this study,the core pathogenesis of systemic lupus alopecia is proposed based on the theory of"deficient Qi stagnation".Deficiency of the liver,spleen,and kidneys is the root cause of the disease.That is,congenital kidney essence insufficiency,acquired spleen-stomach impairment,and liver blood depletion lead to"deficient Qi"with weakened propelling force.Blood stasis blocking the meridians is a symptom of the disease.That is,the stagnation of pathological products such as dampness,phlegm,and toxins causes Qi-blood disharmony and impede their circulation,resulting in the withering and shedding of hair orifices and hair roots due to inadequate essential nutrient supplement.Therapeutic strategies emphasize reinforcing foundational vitality through tri-organ regulation to address deficient Qi-focusing on liver-kidney tonification and spleen-strengthening Qi supplementation-while concurrently eliminating pathogens and unblocking collaterals to resolve stagnation.In terms of relieving stagnation,treatment protocols are tailored to disease phases:heat-clearing and blood-cooling method is used during the acute stage,Qi-tonifying and blood-activating approach is used in the stable period,and essence-replenishing and blood-nourishing method is used in the remission stage,supplemented by Chinese medicinal materials of blood-activating and blood stasis-dispelling,and phlegm-resolving and toxin-eliminating.One supporting case study demonstrates the theory of"deficiency Qi stagnation"as the cornerstone of syndrome differentiation and treatment,validating its guidance value in clinical practice and offering novel insights into the pathogenesis and clinical treatment of the disease.关键词
