Abstract
Adenomyosis refers to a type of disease in which the glands and stroma of the growing endometrium infiltrate into the uterine muscle layer,causing hyperplasia and hypertrophy of the uterine muscle layer.Clinically,it often manifests as progressively worsening dysmenorrhea,menorrhagia,infertility,uterine enlargement,and other symptoms.The pathogenesis of adenomyosis has not yet been fully elucidated.Western medicine primarily employs hormonal palliative therapy and surgical treatment,both of which have certain limitations.Based on its clinical manifestations,Traditional Chinese medicine(TCM)categorizes adenomyosis under the scope of"abdominal mass","dysmenorrhea","menorrhagia",and"infertility."In recent years,TCM has played a significant role in preventing and treating complex and refractory gynecological disorders.TCM treatment can,to some extent,slow the progression of adenomyosis and improve clinical symptoms in patients.This study investigates the pathogenesis,diagnosis,and treatment strategies for adenomyosis through the lens of the TCM principle,"where there is stuffiness and rigidity,there must be latent Yang(阳)",combined with modern Western pathological research.The concept of"stuffiness and rigidity"aligns with the pathological features of adenomyosis,such as hyperplasia and hypertrophy of the uterine myometrium,as well as the clinically progressive worsening of dysmenorrhea.Meanwhile,the"latent Yang"state reflects a dysfunction of Yang Qi's(气)defensive capacity,leading to pathological accumulations of Qi stagnation,phlegm-dampness,and blood stasis.Over time,these factors transform into toxins and heat,depleting healthy Qi while pathogenic factors persist.These mechanisms correspond to the imbalanced local immune microenvironment,hypoxia,and chronic inflammation observed in adenomyotic lesions,where pathogenic factors gradually deplete vital Qi.Building upon this theory,this paper examines the disease progression of adenomyosis,which manifests in three stages:failure of latent Yang to consolidate-constrained latent Yang-deficiency of latent Yang with depletion of vital Qi.To address this pathogenesis,we propose a therapeutic strategy centered on tonifying Qi and warming Yang to strengthen the root,dispersing Yang and resolving stagnation to eliminate pathogenic turbidity and restoring vital Qi and consolidating recovery to prevent relapse.This approach aims to provide a comprehensive TCM-based framework for understanding and treating adenomyosis,integrating classical theory with modern pathological insights.关键词
