超声引导下靶向针刺环咽肌治疗环咽肌失弛缓型吞咽障碍的临床研究
A clinical study on treating cricopharyngeal achalasia type dysphagia by ultrasound-guided targeted acupuncture on the circumpharyngeal muscle

Abstract
Objective:To observe the clinical effect of ultrasound-guided targeted acupuncture on the circumpharyngeal muscle on cricopharyngeal achalasia type dysphagia after stroke.Methods:A total of 86 stroke patients with cricopharyngeal achalasia type dysphagia after stroke were randomly divided into the control group and the treatment group,with 43 cases in each group.Patients in both groups were given routine swallowing function training,including pharyngeal cold stimulation,Mendelssohn manipulation training,feeding training,nutrition and symptomatic support treatment.The treatment group was treated by targeted acupuncture on the cricopharyngeal muscle under dual positioning of ultrasound guidance and balloon on the basis of the treatment in the control group.The clinical efficacy and swallowing function were compared between the two groups.The swallowing X-ray fluoroscopy was used to evaluate the swallowing function of patients in both groups before and after treatment.Results:The total efficiency in the treatment group was significantly higher than that in the control group(P<0.01).After treatment,the VFSS score,swallowing passage time,cricopharyngeal muscle opening condition,and aspiration rate in the two groups were all significantly ameliorated compared with those before treatment(P<0.05),and the amelioration in the treatment group was more significant than the control group(P<0.05).The rates of eating liquid rate,eating paste rate,and eating solid food rate in the treatment group were all significantly higher than those in the control group(P<0.01).The incidence of complications in the control group was 13.95%(6/43),while there were no complications and adverse reactions in the treatment group during the observation period,and the difference between the two groups was statistically significant(P<0.05).Conclusion:Ultrasound-guided targeted acupuncture on the cricopharyngeal muscle shows a good effect on cricopharyngeal muscle achalasia type dysphagia after stroke,which can effectively shorten the swallowing passage time,improve swallowing function,ameliorate the opening state of cricopharyngeal muscle,reduce the aspiration rate and the incidence of complications,improve the ability to eat,provide physiological basis for long-term rehabilitation,and has positive significance for enhancing the rehabilitation confidence of patients and their families and improving the quality of life of patients.关键词
