刘光珍教授从"邪实"分期辨治糖尿病肾病经验
Professor LIU Guangzhen's clinical experience in differentiating and treating diabetic nephropathy by stages from the aspect of"pathogenic excess"
摘要
Abstract
Diabetic kidney disease(DKD)is one of the most common chronic microvascular complications of diabetes,and belongs to secondary renal lesions.In recent years,with the continuous rise in the prevalence of diabetes,the incidence of DKD has also been increasing.Currently,Western medicine lacks ideal strategies to halt the progression of DKD,while traditional Chinese medicine(TCM)has demonstrated unique therapeutic effects and promising clinical prospects in slowing disease progression and relieving clinical symptoms.Professor LIU Guangzhen,with over thirty years of experience in the clinical diagnosis and treatment and research of nephropathy,has proposed the theory of differentiation and treatment of"pathogenic excess"in DKD based on profound clinical experience and theoretical reflection."Pathogens stagnating in the kidney collaterals and forming stasis in the kidneys slightly"is considered as the core pathogenesis of diabetic nephropathy by Professor LIU Guangzhen,and"purging the kidney and eliminating pathogens"is established as the primary treatment principle.The treatment is differentiated into three stages according to the disease course.In the early stage,the pathogenesis is mainly characterized by dryness-heat injuring the kidney and stasis obstructing the kidney collaterals,and the treatment focuses on clearing heat and moistening dryness,and activating blood circulation and resolving stasis.In the middle stage,the pathogenesis is primarily internal accumulation of damp-turbidity and blockage of the kidney collaterals,and treatment aims to eliminate turbidity to promote diuresis,and remove blood stasis to unblock the collaterals.In the advanced stage,the pathogenesis involves internal retention of turbidity-toxin and the formation of slight mass in the kidney,and treatment principles are to unblock Fu internal organs(腑)to discharge turbidity,and resolve toxin to disperse accumulations.This diagnostic and therapeutic system integrates theory,principle,prescription,and medication,and has achieved significant clinical efficacy.关键词
