Effects of Early Asymmetric Ankle Osteoarthritis on Gait and Lower-Limb Biomechanics in Middle-Aged and Older Adults

Abstract
Objective:To investigate the effects of early asymmetric ankle osteoarthritis(AOA)on bilateral gait patterns and lower-limb biomechanics in middle-aged and older adults using three-dimensional gait analysis.Methods:Ninety-eight patients with early asymmetric AOA were recruited as the AOA group,and 85 age-and sex-matched healthy adults without recent lower-limb musculoskeletal disorders were included as controls.Collect baseline information from all participants.Spatiotemporal,kinematic,and kinetic parameters during walking were collected using the SMART-DX 400 three-dimensional gait analysis system.Gait parameters were calculated with BTS-SmartClinic software,and bilateral differences between groups were compared.Results:Compared with the control group,the AOA group showed increased stance phase,double-support phase,and peak hip flexion angle,while swing phase,step length,peak knee extension,ankle dorsiflexion,ankle plantarflexion,the second peak of vertical ground reaction force(V-GRF),and peak ankle plantarflexion moment were significantly reduced(P<0.05).Changes were more pronounced in the affected limb(AFL),which also demonstrated significantly reduced single-support phase,peak hip extension angle,and the first peak of V-GRF.Conversely,the non-affected limb(NAL)exhibited an increased first peak of V-GRF.In addition,the AOA group as a whole was characterized by a reduced cadence(P<0.05).Conclusion:Early asymmetric AOA induces bilateral biomechanical adaptations in middle-aged and older adults to relieve pain and maintain stability.These findings provide a biomechanical basis for early gait intervention in AOA and highlight the importance of bilateral rehabilitation strategies to prevent secondary injuries.关键词
