益气养阴通络方通过调控AMPK/SIRT1通路减轻糖尿病肾病肾损伤的机制研究
Mechanism Research on Yiqi Yangyin Tongluo Formula Alleviating Renal Injury of Diabetic Nephropathy by Regulating the AMPK/SIRT1 Pathway

Abstract
Objective:To explore the potential renal protective mechanisms of Yiqi Yangyin Tongluo Formula(YYT)in db/db mice with diabetic nephropathy(DN).Methods:Twenty-four db/db mice were randomly divided into the model group,irbesartan group(19.5 mg/kg),and YYT group(37.44 g/kg),with 8 db/m mice serving as the normal control group.The model and normal groups received equal volumes of distilled water via gavage for 8 weeks.Parameters assessed included:24-hour urinary total protein(24 h-UTP),fasting blood glucose(FBG),serum blood urea nitrogen(BUN)and creatinine(Scr);renal histopathology evaluated by HE staining,PAS staining,and transmission electron microscopy(TEM);immunohistochemical analysis of podocyte markers(slit diaphragm protein Nephrin and membrane protein Podocin)and autophagy-related proteins(Beclin1 and substrate P62);along with Western blot detection of renal AMPK,p-AMPK,and SIRT1 protein expression.Results:Compared with the normal group,the model group exhibited significantly increased 24 h-UTP,FBG,BUN,and Scr(P<0.05),along with significant renal histopathological al-terations.Renal protein expression of Nephrin,Podocin,and Beclin1 decreased(P<0.05),while P62 increased(P<0.05).The p-AMPK/AMPK ratio and SIRT1 expression were significantly reduced(P<0.05).Compared with the model group,both the irbe-sartan and YYT groups showed significantly decreased 24 h-UTP,FBG,BUN,and Scr levels(P<0.05),ameliorated renal patholog-ical damage to varying degrees,elevated expression of Nephrin,Podocin,and Beclin1(P<0.05),reduced P62 expression(P<0.05),and increased p-AMPK/AMPK ratio and SIRT1 expression(P<0.05).Conclusion:YYT may ameliorate disease progression by activating the AMPK/SIRT1 signaling pathway,enhancing autophagy activity,and attenuating podocyte injury.关键词
