Exploring the"Pattern-Based Group"Diagnosis and Treatment System of Minor Bupleurum Decoction in Treatise on Cold Damage Diseases

Abstract
In Treatise on Cold Damage Diseases,Minor Bupleurum Decoction does not correspond to a single,rigid pattern.Instead,it ad-dresses a"pattern group"characterized by a core pathogenesis of"impaired pivot mechanism of lesser yang,"which manifests with a rich variety of clinical manifestations and follows a dynamic evolutionary pattern.This group possesses its core primary symptoms and peripheral or possible symptoms,exhibiting relatively stable common features alongside individualized variations.It also involves com-plex relationships with disease patterns of other meridians,including transmission,combination,and the need for differentiation.Firstly,the application of Minor Bupleurum Decoction is not confined to the lesser yang domain alone,but it extensively pertains to disease pat-terns involving multiple meridians such as greater yang,yang brightness,reverting yin meridians,and post-illness recovery.Secondly,al-though the derived formulas of Minor Bupleurum Decoction correspond to diverse patterns with varying drug modifications,they all share the core pathogenesis of"impaired pivot mechanism of lesser yang."This embodies the diagnostic and therapeutic principle of"the for-mula adapting to the pattern,and the drugs changing with the disease,"demonstrating the interconnected integrity among the text passa-ges of Treatise on Cold Damage Diseases.Zhang Zhongjing's instruction that"if even one key symptom is present,it is sufficient to apply the formula"is not necessary for all symptoms to be included,"essentially rejects a mechanical listing of symptoms.It points directly to the essential common pathogenesis of"impaired pivot mechanism of lesser yang,"employing a dynamic and holistic mindset to capture the crux of the disease mechanism.Its essence lies in the unity between a highly generalized understanding of the common pathogene-sis and flexible adaptation in clinical practice.The proposal of the"pattern-based group"theory expands the mindset for applying classical formulas,shifting from a framework of"one pattern,one formula"correspondence towards a holistic grasp of the common pathogenesis.Clinically,it is unnecessary to rigidly adhere to a fixed pattern type.The key lies in identifying the core pathogenesis of"impaired pivot mechanism of lesser yang"and observing abnormal states of qi movement,such as stagnation,failure of diffusion,or counter-flow,etc.during the processes of ascending,descending,exiting,and entering.In this way,even when symptomatic presenta-tions are complex and diverse,physicians can manage complexity through simplicity.Using Minor Bupleurum Decoction as a founda-tion,a new paradigm of"treating different diseases with the same method"can be achieved through appropriate tailoring to address various disease syndromes.关键词
