Recovery Time of Parathyroid Function and Its Influencing Factors in Patients with Papillary Thyroid Cancer after Minimally Invasive Thyroidectomy

Abstract

Objective This study aimed to investigate the effect of minimally invasive thyroid surgery on postoperative parathy-roid function recovery in patients with The First Affiliated Hospital of Xinxing Medical Collega(PTC),determining the recovery time and related influencing factors.Methods This study is a cohort study.Clinical data were collected from 110 PTC patients who under-went surgery at The First Affiliated Hospital of Xinxiang Medical College from January 2023 to January 2024,including 55 patients who received minimally invasive surgery and 55 who received traditional open surgery.The data included preoperative thyroid and para-thyroid function indicators,intraoperative parathyroid identification and protection,operative time,postoperative serum calcium and parathyroid hormone(PTH)levels,recovery time of parathyroid function,postoperative complications(e.g.,hypocalcemia),length of hospital stay,and postoperative pain scores.Multivariate Cox regression analysis was used to identify factors affecting parathyroid func-tion recovery,and postoperative indicators were compared between groups.Results The recovery time of parathyroid function was sig-nificantly shorter in the minimally invasive group than in the conventional surgery group[median recovery time:10(8~12)days vs.17(15~21)days,P<0.001].Postoperative serum calcium and PTH levels at 24 and 72 hours were significantly higher in the mini-mally invasive group compared to the traditional group(P<0.001),with a lower incidence of hypocalcemia(15.8%vs.32.5%,P=0.005).Multivariate Cox regression analysis showed that the type of surgery(HR=0.55,95%CI:0.33~0.91,P=0.021),intraop-erative parathyroid protection(HR=0.58,95%CI:0.35~0.95,P=0.037),early postoperative serum calcium levels(HR=1.32,95%CI:1.10~1.60,P=0.002),and operative time(HR=1.12,95%CI:1.05~1.20,P=0.001)were independent factors affect-ing parathyroid function recovery.Additionally,the minimally invasive group had a significantly shorter hospital stay(5.2 vs.7.8 days,P<0.001)and lower pain scores on postoperative days 1 and 3(P<0.001)compared to the traditional group.Conclusions Mini-mally invasive thyroid surgery demonstrates significant advantages in promoting parathyroid function recovery in PTC patients,reduc-ing recovery time and hypocalcemia incidence while shortening hospital stay and minimizing postoperative pain.The type of surgery and intraoperative parathyroid protection are key factors influencing recovery.
