电针结合盆底肌功能训练治疗前列腺癌根治术后尿失禁的临床随机对照研究
A Randomized Controlled Clinical Study on Electroacupuncture Combined with Pelvic Floor Muscle Training for Post-prostatectomy Urinary Incontinence

Abstract
Objective To evaluate the clinical efficacy of electroacupuncture(EA)combined with pelvic floor muscle training(PFMT)in treating radical prostatectomy urinary incontinence(RPUI).Methods A randomized,controlled,single-blind trial was conducted.70 RPUI patients recruited between December 2021 and November 2022 were randomly assigned(1:1)to an EA group or a control group.Both groups received PFMT.The EA group received bilateral needling at Zhongliao(BL-33)and Huiyang(BL-35)acupoints with continuous-wave EA(50 Hz)for 30 minutes per session,twice weekly(with 2-3 days between sessions),the total treatment lasts for 8 weeks.The control group received non-penetrating sham EA at non-acupoints(no current output)with the same treatment schedule.The primary outcome was the change between the total score of International Consultation on Incontinence Questionnaire-Short Form(ICIQ-SF)and the baseline at week 8.Secondary outcomes included ICIQ-SF scores,24-hour(24H)pad test results,and Incontinence-Quality of Life(I-QOL)scores during the treatment period(weeks 0,4 and 8)and follow-up(weeks 12 and 24).Results ①ICIQ-SF scores:At week 8,the EA group showed a significantly greater reduction in ICIQ-SF total score compared to the control group(P<0.05),with lower scores at all timepoints(weeks 4,8,12,and 24)(P<0.05).②24 H pad test:The EA group demonstrated a significant reduction in urine leakage from week 4 onward,with lower values than the control group at all timepoints(P<0.05).③ I-QOL score:The EA group had significantly higher I-QOL total scores from week 8 onward(P<0.05),particularly in the"avoidance and limiting behavior"and"social embarrassment"domains.Conclusion EA combined with PFMT effectively alleviates urine leakage,reduces leakage volume,and significantly improves quality of life in RPUI patients.Its therapeutic effect is superior to that of sham EA combined with PFMT,and it shows a favorable safety profile.关键词
