不同灸法治疗膝骨关节炎的临床研究
Clinical study of different moxibustion methods in treating knee osteoarthritis

Abstract
Objective To observe and compare clinical efficacy differences between needle-warming moxibustion and gentle moxibustion at Xi San Zhen(three knee-related points)in treating knee osteoarthritis.Method Seventy patients with unilateral KOA were randomized into a trial group and a control group,with 35 cases in each group.Both groups underwent regular rehabilitation training.In addition,the trial group received needle-warming moxibustion at Xi San Zhen,and the control group received gentle moxibustion at the same points.Before and after treatment,the two groups were observed for visual analog scale(VAS)score,thigh muscle atrophy index,surface electromyography(sEMG)parameters[integrated electromyography(iEMG),median frequency(MF),muscle contribution rate(CR),and root mean square(RMS)]of the vastus lateralis,rectus femoris,and vastus medialis muscles,and isokinetic muscle strength test parameters[peak torque(PT)and hamstring-to-quadriceps PT ratio(H/Q)].The correlation between the VAS score and isokinetic muscle strength test parameters,as well as the thigh muscle atrophy index,was also observed.Result After the treatment,the VAS score and thigh muscle atrophy index significantly decreased(P＜0.05),and the PT and H/Q increased significantly(P＜0.05)in the trial group;the control group showed a notable decrease in the VAS score(P＜0.05)and a significant increase in the PT(P＜0.05).Statistical significance was observed in between-group comparisons of post-treatment differences in the VAS score,as well as in the isokinetic muscle strength test parameters and thigh muscle atrophy index(P＜0.05).At 60°/s,the iEMG of the vastus medialis muscle increased significantly in the trial group(P＜0.05),and the trial group showed superiority over the control group regarding the IEMG,MF,and RMS(P＜0.05);at 90°/s,the iEMG of the vastus lateralis muscle increased significantly in the trial group(P＜0.05);at 180°/s,the trial group surpassed the control group in comparing the iEMG of the vastus lateralis muscle(P＜0.05).There was a negative correlation between the VAS score and PT of the affected limb before and after the treatment(P＜0.05).Conclusion Needle-warming moxibustion at Xi San Zhen can alleviate pain and muscle atrophy,enhance the strength of peri-knee muscles,and improve extensor muscle strength and muscle balance in KOA patients;the mechanism may be associated with regulating the neuro-muscle pathway and promoting muscle fatigue.关键词
