舌三针联合中药冰刺激治疗卒中后吞咽障碍的疗效观察
Observations on the efficacy of tongue three-needle therapy plus Chinese herbal ice stimulation for post-stroke dysphagia

Abstract
Objective To observe the clinical efficacy of tongue three-needle therapy plus San Zi throat-soothing decoction ice stimulation for post-stroke dysphagia.Method One hundred and twenty patients with post-stroke dysphagia were randomized to three groups(an ice stimulation group,a tongue three-needle group and a combination group),with 40 cases in each group.All the three groups received swallowing function training.In addition,the ice stimulation group received San Zi throat-soothing decoction ice stimulation;the tongue three-needle group,tongue three-needle therapy;the combination group,tongue three-needle therapy plus San Zi throat-soothing decoction ice stimulation.The TCM symptom,the Kubota water swallowing test(WST),the standardized swallowing assessment(SSA)and the swallowing quality of life questionnaire(SWAL-QOL)scores,surface electromyographic indicators of pharyngeal muscles and hyoid-laryngeal complex movement were observed in the three groups before and after treatment.The clinical therapeutic effect and the incidence of adverse reactions were compared between the three groups.Result After treatment,the TCM symptom,WST and SSA scores were lower(P＜0.05),the SWAL-QOL score was higher(P＜0.05),the maximum amplitude of surface electromyography of pharyngeal muscles was higher(P＜0.05),swallowing duration was lower(P＜0.05)and hyoid-laryngeal complex movement range was higher(P＜0.05)in the combination group than in the other two groups.The total efficacy rate was higher(P＜0.05)and the incidence of adverse reactions was lower(P＜0.05)in the combination group than in the other two groups.Conclusion On the basis of swallowing function training,tongue three-needle therapy plus San Zi throat-soothing decoction ice stimulation can effectively relieve the clinical symptoms,increase the mobility of the hyoid-laryngeal complex and improve swallowing function in treating post-stroke dysphagia.Its therapeutic effect is better than that of tongue three-needle therapy or San Zi throat-soothing decoction ice stimulation alone.关键词
