基于"筋骨肉并重"理论独活寄生汤内服联合中医定向透药疗法治疗肝肾亏虚型膝骨关节炎的临床观察

Abstract
Objective:To observe the clinical efficacy of Duhuo Jisheng Decoction(Pubescent Angelica and Taxillus Decoction)oral administration combined with TCM directional drug penetration therapy and functional exercise in treating knee osteoarthritis(KOA)with liver-kidney deficiency pattern.Methods:Eighty-four KOA patients with liver-kidney de-ficiency pattern were randomly divided into two groups.Both groups received functional exercise;the control group(n=42)received oral celecoxib capsules combined with external Shexiang Huoxue Huayu Plaster(Musk Blood Activating and Stasis-resolving Plaster),while the observation group(n=42)received Duhuo Jisheng Decoction oral administration com-bined with TCM directional drug penetration therapy.Treatment duration was 4 weeks for both groups.Outcome measures included Visual Analogue Scale(VAS)for knee pain,Knee Injury and Osteoarthritis Outcome Score(KOOS),flexion/ex-tension muscle strength,TCM syndrome scores,and total effective rate.Results:Post-treatment VAS scores significantly decreased in both groups versus pre-treatment,but intergroup differences were not statistically significant(P>0.05).KOOS scores,knee flexion/extension muscle strength,and TCM syndrome scores improved in both groups,with superior efficacy in the observation group(P<0.05,P<0.01).The total effective rate in the observation group(90.47%)was significantly higher than in the control group(73.81%)(P<0.05).Conclusion:Duhuo Jisheng Decoction oral administration combined with TCM directional drug penetration therapy and functional exercise demonstrates significant clinical efficacy for KOA with liver-kidney deficiency pattern,effectively alleviating knee pain,enhancing flexion/extension muscle strength to stabi-lize the knee joint,improving functional mobility,and mitigating systemic symptoms.关键词
