Abstract
Objective To analyze the correlation between serum D-dimer(D-D)and interleukin-6(IL-6)levels and the severity of refractory Mycoplasma pneumoniae pneumonia(RMPP)in children and its predictive value for prognosis.Methods A total of 200 children with RMPP admitted to the Department of Pediatrics,the First Affiliated Hospital of Baotou Medical College,Inner Mongolia University of Science&Technology from January to December 2024 were selected as the case group,and 100 children who underwent physical examination during the same period were selected as the control group.According to the clinical pulmonary infection score(CPIS),the children in the case group were divided into mild group(n=110)and severe group(n=90).According to the clinical outcome after 28 days of treatment,the children were divided into good prognosis group(n=135)and poor prognosis group(n=65).The clinical data of the children were collected,and the correlation between the serum D-D and IL-6 levels on admission and the severity of the disease was analyzed.The influencing factors of poor prognosis were analyzed,and the receiver operating characteristic(ROC)curve was drawn to evaluate the predictive value of D-D and IL-6 for poor prognosis in children with RMPP.Results The levels of serum D-D and IL-6 in the mild group and the severe group were higher than those in the control group(P<0.05).The levels of serum D-D and IL-6 in the severe group were higher than those in the mild group(P<0.05),and were positively correlated with the CPIS score(r=0.713,0.689,P<0.05).The levels of D-D and IL-6 in the poor prognosis group were higher than those in the good prognosis group(P<0.05).Multivariate analysis showed that D-D(OR=5.562,P=0.008)and IL-6(OR=4.276,P=0.003)were independent influencing factors for poor prognosis in children with RMPP.The ROC curve showed that the area under the curve(AUC)of D-D and IL-6 for predicting the prognosis of children with RMPP was 0.871 and 0.854 respectively,and the AUC of the combined prediction was 0.930,which was greater than that of the single prediction(P<0.05).Conclusion The levels of serum D-D and IL-6 can reflect the severity of RMPP in children and have a high predictive value for poor prognosis.The clinical attention should be focused on these indicators.关键词
