Influence of traditional Chinese medicine constitution on the mechanical bearing capacity of femoral head in patients with femoral head necrosis based on imaging and finite element analysis

Abstract
Objective It is to explore the mechanical bearing capacity of femoral head in femoral head necrosis patients with different traditional Chinese medicine constitutions.Methods Twenty-four patients with femoral head necrosis at Stein-berg stageⅡclassified as Yang deficiency,damp-heat,or blood stasis constitutions and enroll 9 patients with normal femo-ral heads,who visited the Orthopedics Outpatient Department of Guang'anmen Hospital of China Academy of Chinese Med-ical Sciences from January 2022 to January 2024 were enrolled by a retrospective analysis.There were 7 patients(10 hips)in the Yang deficiency group,9 patients(10 hips)in the damp-heat group,8 patients(10 hips)in the blood stasis group,and 9 patients(15 hips)in the normal group.The mean CT densities at the middle plane of coronal position and maximum necrosis plane,along with mechanical parameters(maximum total displacement,maximum displacement in the Z-direc-tion,maximum equivalent stress,and failure coefficient)in finite element analysis were compared among all groups.Re-sults There were significant differences in the mean CT density values at the middle plane of coronal position and maximum necrosis plane(F=11.693,P<0.001;F=9.864,P<0.001),maximum equivalent stress(F=5.305,P=0.003),and failure coefficient(χ2=28.486,P<0.001)among the four groups,but there were no significant differences in the maximum total displacement(χ2=2.984,P=0.394)and maximum displacement in the Z-direction(χ2=2.973,P=0.396)among the four groups.The mean CT density values at the middle plane of coronal position and maximum necrosis plane were significantly higher in all femoral head necrosis groups of different constitutions than those in the normal group(all P<0.05),the mean CT density value at the maximum necrosis plane was significantly higher in the blood stasis con-stitution group than that in the Yang deficiency constitution group(P<0.05);the maximum equivalent stresses were sig-nificantly higher in both the blood stasis constitution group and Yang deficiency constitution group than than in the normal group(both P<0.05);the failure coefficient in the Yang deficiency constitution group was significantly higher than those in the blood stasis constitution group,damp-heat constitution group and normal group(all P<0.05),while the failure co-efficient in the blood stasis constitution group was significantly lower than that in the normal group(P<0.05).Conclu-sion There are differences in the mechanical distribution and bearing capacity of femoral head among femoral head necrosis patients with different traditional Chinese medicine constitutions.Patients with blood stasis constitution have higher mechan-ical strength of femoral head,those with Yang deficiency constitution have reduced load-bearing capacity of trabecular bone which prone to collapse,thereby timely constitutional adjustment should be administered to prevent femoral head collapse.关键词
