Abstract

Objective It is to explore the distribution characteristics of traditional Chinese medicine(TCM)syndrome elements in patients with type 2 diabetes mellitus(T2DM)complicated with vascular complications,and to explore the pre-dicting values of expressions of serum microRNA-155(miR-155),microRNA-21(miR-21)for these patients.Methods A total of 147 patients with T2DM treated in the Endocrinology Department of Dongzhimen Hospital of Beijing University of Chinese Medicine from April 2019 to April 2021 were selected,and were divided into a vascular complication group(n=102)and non-vascular complication group(n=45)according to the presence or absence of vascular complications.The syndrome characteristics(the TCM four diagnostic information collected via the TCM Syndrome Questionnaire were ana-lyzed),general data,biochemical indicators(including fasting blood glucose,glycated hemoglobin,total cholesterol,tri-glycerides,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol),and serum expressions of miR-155,miR-21,and miR-16 in the patients were compared between the two groups.Single-factor and multi-factor logistic regres-sion analysis were performed to evaluate whether miR-155,miR-21 and miR-16 were associated factors influencing the oc-currence of vascular complications in T2DM patients.Receiver operating characteristic(ROC)curve was constructed to as-sess the efficacies of miR-155,miR-21,miR-16,and their combination in predicting vascular complications in T2DM pa-tients.Results The distribution of disease patterns was primarily characterized by Yin deficiency,followed by Qi deficien-cy,excessive heat,and phlegm-dampness in patients of the non-vascular complication group,while the distribution of dis-ease patterns was primarily characterized by Qi deficiency,followed by Yin deficiency,blood stasis,and phlegm-damp-ness.The patients in the vascular complication group were significantly older,and their disease durations were significantly longer than those in the non-vascular complication group(all P<0.05),the prevalence of hypertension,serum levels of total cholesterol,triglycerides,and low-density lipoprotein cholesterol,as well as the relative expressions of miR-155 and miR-16 were significantly higher,while the relative expression of miR-21 was significantly lower than those in the non-vas-cular complication group(all P<0.05).Multivariate logistic regression analysis revealed that age≥60 years,disease du-ration≥5 years,hypertension,total cholesterol≥5.355 mmol/L,triglycerides≥1.720 mmol/L,low-density lipoprotein cholesterol≥3.295 mmol/L,miR-155≥1.910,miR-21<0.380,and miR-16≥37.140 were independent risk factors for vascular complications in T2DM patients(all P<0.05).ROC curve analysis revealed that miR-155,miR-21,and miR-16 all had predictive efficacy for vascular complications in T2DM patients,with areas under the curve(AUC)of 0.771,0.716,and 0.737,respectively,the area under the curve of combined predicting vascular complications in T2DM patients with above three serum markers was 0.942.Conclusion Qi deficiency and Yin deficiency run through the whole process of diabetic vascular lesions,and blood stasis and phlegm dampness are the key pathological factors of vascular lesions;miR-155,miR-21,and miR-16 are independent risk factors for vascular complications in patients with T2DM and have predic-tive value for the occurrence of vascular complications.
