Efficacy of Sanhuang Yishen granules on diabetic kidney disease and its effects on expressions of TGF-β1,cAMP and PKA

Abstract
Objective It is to observe the efficacy of Sanhuang Yishen granules in the treatment of patients with stage 2-3 chronic kidney disease(CKD)of diabetic kidney disease(DKD)with syndrome of Qi-Yin deficiency and blood stasis,and to explore its potential mechanisms of action.Methods Sixty patients with stage 2-3 CKD of DKD with syndrome of Qi-Yin deficiency and blood stasis treated at the outpatient clinic of Nephrology Department of Fangshan Hospital of Beijing U-niversity of Chinese Medicine from October 2022 to April 2023 were randomly divided into two groups by a random number table,with 30 cases in each group.The treatment group was treated with conventional western medical therapy plus oral ad-ministration of Sanhuang Yishen granules,while the control group was treated with conventional western medical therapy plus oral administration of Keloxin capsule,both groups were treated for 8 weeks.The renal function-related indicators[24-hour urine protein quantification,serum creatinine,blood urea nitrogen,estimated glomerular filtration rate(eGFR)]and renal fibrosis-related indicators[serum transforming growth factor-β1(TGF-β1),cyclic adenosine monophosphate(cAMP),protein kinase A(PKA)],blood lipid indicators[triglycerides(TG),total cholesterol(TC),low-density lip-oprotein cholesterol(LDL-C)]and scores of traditional Chinese medical syndrome were compared between the two groups before and after treatment,the occurrences of dverse reactions in the two groups were statistically analyzed.Results Twen-ty-seven patients in the treatment group and 25 ones in the control group completed the study.The level of serum creatinine in the treatment group was significantly lower,while eGFR was significantly higher than those before treatment and in the control group(all P<0.05),there were no significant differences in the level of serum creatinine and eGFR between be-fore and after treatment in the control group(both P>0.05),there were no significant differences in24-hour urine protein quantification,blood urea nitrogen,and blood lipid indicators between the two groups before and after treatment(all P>0.05).The serum levels of TGF-β1 in both groups were significantly lower(both P<0.05),while the serum levels of cAPM and PKA were significantly higher than those before treatment(all P<0.05),however,there were no statistically significant differences in these indicators between the two groups(all P>0.05).The scores of fatigue,dry mouth,dizzi-ness,facial and limb edema,lower back and knee weakness,and constipation were significantly lower after treatment than those before treatment in the treatment group(all P<0.05),and the scores of these symptoms except for facial and limb e-dema were significantly lower than those in the control group(all P<0.05).No significant changes were observed in liver function,complete blood count,or electrocardiogram after treatment in either group.Conclusion Sanhuang Yishen granules can improve renal function and clinical symptoms of patients with stage 2-3 CKD of DKD with syndrome of Qi-Yin deficien-cy and blood stasis,the mechanism may be related to regulation of expressions of TGF-β1,cAMP,and PKA and inhibition of renal fibrosis.关键词
