三种新型复合炎症标志物与骨质疏松症及骨折的研究进展
Research progress on three novel composite inflammatory markers in osteoporosis and fractures

Abstract
Osteoporosis is a systemic metabolic bone disease characterized by reduced bone mass,destruction of bone microstruc-ture,and increased risk of fractures.Chronic inflammation plays a key role in its pathogenesis.The systemic immune-inflammation index(SII),the systemic inflammation response index(SIRI),and the aggregate index of systemic inflammation(AISI),as novel inflammatory markers calculated based on blood routine parameters(neutrophils,lymphocytes,monocytes,platelets),have received widespread attention in osteoporosis and fracture research in recent years due to their simplicity,low cost,and ready availability.The systemic inflammatory state reflected by these three factors influences bone-metabolic imbalance by modulating the TNF-α/NF-κB and RANKL/RANK/OPG signaling pathways,thereby altering lymphocyte-subset balance,myeloid-cell differentiation,and platelet func-tion:the systemic inflammatory burden reflected by SII is significantly correlated with decreased bone density and fracture risk in post-menopausal women;SIRI can independently predict osteoporosis and fracture occurrence in elderly hypertensive patients;AISI shows potential value in assessing bone density in perimenopausal women.Clinical studies have confirmed that the predictive efficacy of these markers is generally better than that of traditional single inflammatory indicators,but there are differences in the critical values under different populations(such as postmenopausal women,type 2 diabetes patients)and research purposes(diagnosis of osteoporosis/pre-diction of fractures).In general,SII,SIRI,and AISI provide new ideas for early screening and risk assessment of osteoporosis and fractures,but their clinical application standards still need further clarification through large-scale prospective studies.关键词
