PIVKA-Ⅱ、AFP、RPR联合应用在乙肝病毒相关性肝细胞癌中的早期诊断价值
Value of the combination of PIVKA-Ⅱ,AFP,and RPR in the early diagnosis of hepatitis B virus-associated hepatocellular carcinoma

Abstract
Objective To explore the value of the combined application of abnormal prothrombin(PIVKA-Ⅱ),alpha-fetoprotein(AFP),and red blood cell distribution width-to-platelet ratio(RPR)in the early diagnosis of hepatitis B virus-related hepatocellular carcinoma(HBV-HCC).Methods A total of 45 HBV-HCC patients(all were TNMⅠ-Ⅱ)admitted to the People's Hospital of Guigang City from July 2022 to March 2023 were enrolled as case group.And the normal control group(71 healthy people)and cirrhosis control group(45 patients with hepatitis B cirrhosis)were selected as controls.The serum PIVKA-Ⅱ,AFP levels,and RPR values of the three groups were measured.The diagnostic efficacy of different indi-cators alone and in combination for HBV-HCC were compared.The relationships among indicators and the diagnostic advan-tages of their combined application were further analyzed by relevant diagrams.Results The PIVKA-Ⅱand AFP levels,as well as the RPR values,were significantly higher in the case group than in the normal control group and cirrhosis control group,and the RPR value was higher than the normal control group but lower than the cirrhosis control group(P<0.05).The sensitivity of PIVKA-Ⅱ,AFP,and RPR for detecting HBV-HCC separately were 93.3％,93.3％,and 80.0％,respectively,and specificity was 98.3％,96.6％,and 56.9％,respectively.The sensitivity of the combined detection increased to 97.8％and specificity was 100.0％.Based on column chart and calibration curve analysis,the combined diag-nostic model demonstrated good predictive performance and accuracy.Conclusion The combined application of PIVKA-Ⅱ,AFP,and RPR can improve the sensitivity of early HBV-HCC diagnosis and holds significant clinical value.关键词
