疑难配血患者红细胞输注无效的影响因素及预测模型的构建
Influencing factors of ineffective red blood cell transfusion in patients with difficult blood matching and construction of a prediction model
摘要
Abstract
Objective To analyze the influencing factors of the effect of red blood cell transfusion in patients with difficult blood matching,and to construct a prediction model for them.Methods A retrospective analysis was conducted on the clinical data of 105 patients with difficult blood matching admitted to the First People's Hospital of Qinzhou.Based on the efficacy of red blood cell transfusion,patients were divided into effective group(55 cases)and ineffective group(50 cases).Differences of indicators such as general data,serological characteristics,blood biochemistry,and routine blood test between the two groups were analyzed by univariate analysis.Furthermore,multivariate logistic regression analysis was used to analyze the risk factors of ineffective red blood cell transfusion in patients with difficult blood matching,and a risk prediction model for ineffective transfusion was constructed.Results Univariate results showed that there were statistically significant differences in age,transfusion history,fever,infection,blood diseases,direct antiglobulin test(DAT),antibody screening,white blood cell count,hemoglobin,and total bilirubin between the two groups(P<0.05).Multivariate logistic regression analysis showed that transfusion history,fever,infection,positive antibody screening,and low hemoglobin levels were independent influencing factors for ineffective red blood cell transfusion in patients with difficult blood matching(P<0.05).A column chart prediction model for ineffective red blood cell transfusion in patients with difficult blood matching was constructed.After verification,the AUC of this model was 0.970,which indicated that the prediction performance of this model was good.Conclusion A history of transfusion,fever,infection,positive antibody screening,and low hemoglobin levels are associated with a relatively high risk of ineffective red blood cell transfusion in patients with difficult blood matching.A predictive model constructed based on these risk factors provides a scientific basis for the prevention and treat-ment of ineffective transfusion in such patients.关键词
