Abstract
Objective To explore the value of multi-slice spiral CT(MSCT)in predicting WHO/ISUP pathological grade of clear cell renal cell carcinoma(ccRCC)before operation.Methods 68 patients with ccRCC diagnosed by histopathology after operation were retrospectively collected.All patients underwent abdominal CT plain scan plus three-phase enhanced scan before operation.According to WHO/ISUP classification,GradeⅠandⅡwere included in low grade group(49 patients),GradeⅢandⅣwere included in high grade group(19 patients).The general clinical data and MSCT signs of the two groups of patients were collected and compared.Logistic regression was used to analyze the influencing factors of different WHO/ISUP grades of ccRCC,and ROC curves were drawn to evaluate the diagnostic efficacy of the two groups in diagnosing WHO/ISUP grades of ccRCC.Results There were statistically significant differences in related clinical symptoms,maximum diameter,morphology,lobulation sign,renal sinus invasion,intratumoral artery and peritumoral vein between high and low grade ccRCC patients(P<0.05).Logistic regression results showed that intratumoral artery and peritumoral vein were independent predic-tive indicators.ROC curves showed that the AUC of combined model for predicting ccRCC WHO/ISUPgrade(0.897)was higher than that of single index(the AUC of intratumoral artery and peritumoral vein were 0.828 and 0.803,respectively).Conclusion MSCT signs are closely related to WHO/ISUP grade of ccRCC.Intratumoral artery and peritumoral vein are inde-pendent redictor of ccRCC WHO/ISUP grade.The combination model of MSCT and peritumoral vein can accurately predict the pathological grade of ccRCC before operation,which can provide basis for further clinical treatment of ccRCC.关键词
