Abstract

Objective:To assess the feasibility of using transorbital ultrasound multiparameter evaluation for detecting and evaluating increased intracranial pressure(ICP),and to construct a predictive model for rapid and accurate clinical assessment.Methods:We prospectively selected 136 patients from the neurology department,scheduled for lumbar puncture.Before the procedure,we measured optic nerve ultrasound parameters,including optic nerve sheath diameter(ONSD),ONSD to eyeball transverse diameter ratio(ONSD/ETD),peak systolic velocity(PSV),end-diastolic velocity(EDV),and resistance index(RI)of the central retinal artery.Based on lumbar puncture results,patients were divided into increased ICP and ICP normal groups.We analyzed the correlation between parameters and ICP,and the diagnostic consistency of each parameter.ROC curves were used to identify optimal thresholds,and multivariate binary Logistic regression was applied to construct a predictive model for ICP elevation.Results:ONSD,ONSD/ETD,and RI were significantly higher in the increased ICP group,while PSV and EDV were lower(P<0.001).ONSD,ONSD/ETD,and RI showed positive correlations with ICP,while PSV and EDV showed negative correla-tions(P<0.001).The consistency between parameters and lumbar puncture pressure was significant(P<0.001).A multimodal pre dictive model was developed with the formula:Logit(P)=1.94×ONSD-84.6×ONSD/ETD+0.79×PSV-13.8×RI+11.05.Conclusions:Transorbital ultrasound provides a non-invasive,rapid,and accurate method for increased ICP assessment.A multiparameter ap-proach offers higher diagnostic efficacy than single parameters and may become a key strategy for non-invasive ICP monitoring.
