常规超声联合弹性成像及BRAFV600E基因预测甲状腺乳头状癌腺外侵犯的效能及相关特征分析
Predicting extrathyroidal extension in papillary thyroid carcinoma:efficacy and relevant characteristics of combined conventional ultrasound,elastography,and BRAFV600E gene

Abstract
Objective:To explore the efficacy of conventional ultrasound combined with elastography and BRAFV600E gene testing in preoperatively predicting extrathyroidal extension(ETE)in papillary thyroid carcinoma(PTC),and to analyze relevant clinical and ultrasonographic characteristics.Methods:Clinical data,ultrasound features,elastography parameters,and BRAFV600E gene results obtained from ultrasound-guided fine-needle aspiration cytology(US-FNAC)of 287 patients with surgi-cally and pathologically confirmed PTC were retrospectively analyzed.Patients were divided into an ETE group and a non-ETE group based on pathological results.Characteristics significantly associated with ETE(P<0.05)were screened using univari-ate analysis.Subsequently,independent predictors were identified using multivariate logistic regression analysis.The predictive efficacy of individual characteristics and combined characteristics was evaluated using the area under the receiver operating characteristic curve(AUC).Results:Univariate analysis revealed 13 characteristics significantly associated with ETE(P<0.05),in-cluding age,gender,maximum diameter,multifocality,composition,aspect ratio,microcalcifications,relationship to the capsule,VTI grading,SWVmax,SWVmin,SWVmean,and BRAFV600E gene status.Multivariate regression identified male gender,maximum di-ameter>10 mm,multifocality,tumor contact with the capsule,VTI grade≥5,and BRAFV600E mutation as independent risk fac-tors for predicting ETE.The combined predictive model using these independent factors demonstrated the highest efficacy(AUC=0.923),outperforming any single characteristic(Gender AUC=0.606,Maximum diameter AUC=0.690,Multifocality AUC=0.606,Relationship to capsule AUC=0.759,VTI grading AUC=0.799,BRAFV600E AUC=0.634).Conclusion:Male gender,maxi-mum diameter>10 mm,multifocality,tumor contact with the capsule,VTI grade≥5,and BRAFV600E mutation are useful for predicting ETE in PTC patients.The combined use of these factors provides the highest predictive value.关键词
